Schedule for all positions: M-F, 8a â 4p, no weekends & no holidays
Location: PACE centers and homecare throughout North Shore & Merrimack Valley, MA

General Summary:
Serves as a member of the PACE Interdisciplinary Team. Participates as a member of the Interdisciplinary Team in the planning, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care. Demonstrates proficiency in administering nursing care in a professional and respectful manner with the goal of assisting frail elders to remain living in the community for as long as they can do so safely.
Qualifications: Registered Nurse currently licensed in the Commonwealth of MA. Ideally 1+ years of experience with the geriatric population, however new grads and those Nurses interested in caring for the geriatric population are encouraged to apply. General Summary:
Functions as an integral part of an enrollment team to prioritize, organize, and adapt the PACE enrollment process to meet potential participants' individualized needs and to ensure census goals are achieved. Works as a clinical liaison between prospective participants/caregivers, the PACE enrollment team, the PACE interdisciplinary Team (IDT) and referral sources/other community providers.
Qualifications: Associate's or Bachelor's degree in Nursing, or completion of a Nursing Diploma Program. Licensed as a Registered Nurse in the Commonwealth of Massachusetts. 1+ years of experience in a geriatric healthcare and/or adult mental health setting. Advanced computer skills, including Microsoft Office, Customer Relationship Management, and Electronic Medical Records software. Ability to communicate effectively with and establish working relationships with various medical licensed personnel.
